Yeah - top side ground is a double edge sword - if you use it carefuly you can benefit from it, if not - you can "cut" yourself badly... Careful use of vias is essential!

...and/or shunt capacitors and coils (filters, matching networks), bypass capacitors. SMT attenuators, MMICs etc.

Rule 2 is absolutely wrong. Topside ground doesn't count as RF ground. The RF ground is the plane used as the reference for your microstrip or stripline traces. Device ground pins need to be connected to the RF ground plane with the lowest inductance possible. This means multiple vias directly to the ground plane. Forget about topside ground - it's worthless for RF.

all very obvious, AND ALL HAVE CAUGHT ME OUT IN THE PAST. this is my new print out on the wall, a great concise set of rules to be followed,
